We are seeking a compassionate, driven, and resilient individual to join our Leaving Care Service as a Leaving Care Worker. In this rewarding role, you will be instrumental in supporting young people aged 16 to 25 as they transition from care into independent adulthood.

About the Role

As a Leaving Care Worker, you will act as a trusted point of contact and key coordinator of support for care leavers, ensuring that each young person receives tailored guidance and access to the services they require. You will work closely with partner agencies, families, and the young person's support networks to develop and implement plans that promote successful outcomes in education, employment, housing, health, and overall wellbeing.

You will be part of a dynamic team committed to delivering timely, effective, and empathetic support, including contributing to a duty rota to offer guidance and crisis response when needed. This post demands flexible working, including evenings and weekends.

Key Responsibilities

  • Managing a caseload and delivering direct, person-centred interventions

  • Developing, reviewing, and implementing Pathway Plans

  • Acting as Lead Professional when appropriate

  • Supporting young people to access and sustain accommodation, education, training, and employment

  • Facilitating positive engagement with support networks and external agencies

  • Promoting independence and reducing risks associated with offending, anti-social behaviour, or mental health challenges

  • Maintaining accurate records and participating in supervision and professional development

About You

To succeed in this role, you will have:

  • An NVQ Level 3 qualification (or equivalent)

  • Demonstrable experience or understanding of the needs of care leavers aged 16 to 25

  • Strong interpersonal and organisational skills

  • A full UK driving licence and access to a vehicle for work

  • A solid understanding of safeguarding and child development

  • The ability to work across agencies and with diverse professional teams

  • A commitment to equality, diversity, and inclusion
