Care Coordinator - Reablement Service

About the Role
We are seeking a dedicated and organised Care Coordinator to join a dynamic Reablement Service within Adult Social Care. This role focuses on supporting individuals to regain their independence and confidence, enabling them to live safely and comfortably in their own homes.
The Care Coordinator will play a vital part in delivering a high-quality reablement service - coordinating care, assessing needs, and supporting service users to achieve their personal goals.
Key Responsibilities
Conduct risk assessments and disseminate findings appropriately.
Set out reablement goals and develop action plans with service users and their carers.
Coordinate and support the work of Home Care Enablers.
Monitor, review, and record users' progress toward achieving independence.
Commission care from external agencies where required.
Liaise effectively with community health and social care services, GPs, hospitals, and voluntary organisations.
Maintain accurate and up-to-date case records, including use of relevant IT systems.
Assist senior management with prioritising and allocating workloads within the team.
Take initiative when dealing with emergencies and ensure compliance with Health and Safety requirements.
About You
The ideal candidate will have:
Experience working within health or social care.
Strong understanding of the concept of reablement or enablement.
Excellent communication and organisational skills.
The ability to set goals, create action plans, and manage progress effectively.
Proficiency in maintaining accurate electronic records and databases.
A collaborative approach to working with colleagues and external partners.
Flexibility to work some unsocial hours, including weekends and bank holidays when required.
A driving licence and access to a vehicle (desirable).
Qualifications
Educated to degree level or equivalent.
Experience supervising staff is desirable.
Willingness to undertake Trusted Assessor training if not already qualified.
