SEND Case Co-ordinator

We are currently looking for a SEND Case Co-Ordinator for a role in Yate. As a Special Educational Needs and Disability (SEND) case worker you will be part of an integrated multi-disciplinary service, comprising; social workers, occupational therapists, SEND education and portage specialists and the transitions and travel training service. The service is structured into four teams. 0-18 social care team, 16 – 25 social care team, transitions team and the SEND education team.
You will be accountable for co-ordination of Education, Health Care plans (EHCP). You will be required to act as the key point of contact during the development of an EHC Plan and ongoing reviews for the child, young person and family, you will oversee any activities that are required to assess needs, develop a plan to meet any outcomes specified as part of the EHC integrated assessment You will work to ensure that the EHC plan is focused on the future needs of the young person to help them towards a successful adulthood. The plan will cover all the areas of a young person’s life, not just those aspects concerned about being in a formal education setting. You will work in partnership with the young person and their parents/carers to help them to plan towards the young person’s adult life and promote independence, choice and control over all aspects of their like. This will include having clear outcomes in the EHC plan that addresses the young person’s aspirations around employment and independent living.
You will be supervised by the Team Leader SEND, you will also have access to advice and guidance from the Senior SEND Co-ordinators within the team as required.
You will model good practice and have knowledge and understanding of legislation which specifically relates to disabled children, young people and their families. You will participate in development activity as required to maintain and develop your skills and knowledge.
You will be required to work within statutory time scales to complete and review plans to the required standard.
You will act as a key contact for named service users and schools and education providers.
You will provide service users, schools, health, teams within the service and other agencies with general advice and guidance about the service and resolve issues.
You will attend meetings, such as annual review meetings and may be required to chair meetings such as person centred planning meetings, calling upon support from Team Leaders/ Team Manager where required.
You will provide information and support around the Local Offer and will be expected to stay up to date with the Local offer.
You must be vigilant to safeguarding issues and must refer to a senior when you have any concerns about the safety or welfare of a service user.
You will hold informal mediation meetings to discuss panel outcomes with families
You will be accountable for keeping accurate and up to date records and update systems as required.
You will participate in audits and other activity under the service quality assurance framework.
You will hold caseload responsibility and will prepare well written outcomes focused reports for panels
