Social Care Assessor

An exciting opportunity has arisen for a Social Care Assessor to work in Greenwich within the Adult’s Social Care Sector. The postholder will l be providing information and advice, completing Care Act Assessments, signposting to other teams and community resources, processing referrals from all emergency services, the public, other local authorities, carers and families (and of course self-referrals), liaising with community and hospital teams, resolving disputes between receivers and providers of social care. You will also have opportunities to create/amend care and support plans and complete face to face visits in the community although the role is largely office-based (there may be opportunities to work from home).
Purpose of Job:
Visiting Adults and Older People and/or their carer’ s in their place of residence (or other setting), and work in partnership with them (and others) to assess their social care needs, identify risks, propose solutions, provide information and advice and facilitate the development and implementation and/or review of care and support plans (and support plans).
Following suitable training, assessing for, recommending and making arrangements for the delivery and collection of simple equipment, to demonstrate its purpose and appropriate use and make appropriate personalised adjustments within the manufacturer’s guidelines. Acquiring and maintaining a knowledge of current social care legislation, national and local guidance and best practice in social care.
Ensuring that all work complies with Council policies and procedures and that statutory obligations are met.
The quality and prioritisation of your own work, following direction from your Line Manager, and for alerting your Line Manager at the earliest opportunity to any case issues, caseload issues, safeguarding concerns, practice issues, issues of unaddressed risk that will adversely impact the health and wellbeing of an individual or their carer, or issues that may impact the good reputation of the department or the local authority.
